Job Title: Facilitator/Care Coordinator
Reports to: Program Manager
Salary Range: $36K - $38K
Facilitator/Care Coordinator will support OIC's Sexual Risk
Avoidance Education Program (SRAE), which includes evidence-based
prevention curricula : Love Notes and Relationship SMARTS Plus
(RSP) paired with positive youth development and trauma informed
educational approaches serving at-risk youth ages 10 to 21. The
Facilitator/Care Coordinator is responsible for delivering program
through instructions with fidelity, collecting performance data and
implementing positive youth development activities, supporting
community saturation through community engagement and
outreach in multiple settings.
Essential Duties and Responsibilities:
--- Effectively deliver evidence-based curricula for program
participants, reinforcing age- appropriate language and medically
accurate instructional strategies.
--- Assist the Community Outreach & Mobilization Coordinator in
identifying and securing
middle and high schools, alternative schools, and community-based
sites to host SRAE
educational programming for project youth.
--- Coordinate strengths-based youth development activities, which
include the following but not limited to: role plays, role model
presentations, and referrals to community resources.
--- Employ trauma-informed educational practices.
--- Ensure that program activities remain inclusive, promote
cultural and LGBTQ sensitivity and enforce anti-harassment
--- Enter enrollment forms, classroom attendance and fidelity
monitoring logs into program database on a daily/weekly basis.
--- Assist in coordinating and attending special events such Youth
Meetings and assist in the program's community saturation and
for youth, families, and community stakeholders.
--- Assist team in coordinating educational support meetings for
parents of project youth, which include encouraging parental
involvement, facilitating relevant topics of interest, securing
guest speakers, and providing resources and referrals as
--- Assist Lead Facilitator/Care Coordinator in coordinating
special events and positive youth development activities, which
include the following: role plays, role model
presentations, and end-of-program recognition events.
--- Assist Management and Lead Facilitator/Care Coordinator with
the Youth Leadership
--- Routinely review the most current CDC, NIH and state and local
vital stats to ensure program materials adhere to medical accuracy
and report any findings to the Programs Manager.
--- Conduct research and develop/update program materials for
--- Prepare monthly reports, develop and maintain project
--- Attend professional development trainings and internal and
community meetings deemed necessary by the Program Manager, and/or
Lead Facilitator/Care Coordinators. Attend monthly program team
--- Follow all requirements and guidelines of the program funder
under the guidance of
--- Share program and community resources with other team members
--- Submit monthly statistics to Lead Facilitator/Care Coordinator
in a timely manner as required.
--- Complete Exit Surveys and Site Implementation Issue forms in
accordance to policy.
--- Attend scheduled meetings deemed necessary by the Program
--- Provide support for program sustainability and resource
Minimum Qualifications: Minimum Bachelor's Degree in Education,
Training, Psychology or related degree. Minimum of 1-3 years of
relevant experience in education, counseling and/or providing
training, mentoring, support to at-risk youth. Because of the
nature of our work with youth, all applicants must be able to pass
Level II Background Screening as required by the Broward County
Other Skills and Abilities:
--- Competency working with adolescents. Strong presentation and
classroom management skills with a proven ability to engage and
retain program participants.
--- Ability to build and maintain professional working
relationships with other team
members, school personnel, community/service providers. Knowledge
of adolescent pregnancy determinants and youth development
--- Ability to work with individuals from diverse cultural, ethnic
and social groups. Bilingual skills (English, Spanish, French,
Creole) are a plus.
--- Strong organizational and analytical skills. Must be computer
literate with proficiency in
Microsoft Word, Excel, PowerPoint, and Prezi. Qualifications may be
proven through exam. Strong oral and written communication

a team member.

--- Performing the duties of this position requires the ability to
walk, sit, stand, reach, talk, hear and lift presentation
materials, including equipment. This position will require
independent travel between local schools and other venues. Must
Driver's License and insurance coverage on personal vehicle at all
