Job Description Job Description Summary Disease management Care Navigators work with the Director of Medical affairs and Chief Medical officer to coordinate the care of the Disease Management (DM) patients. Additionally, they assist the center administrators, Complex case Care navigators, and PCPs to engage the DM patients and ensure they receive timely, disease- and acuity-specific care. Duties and Responsibilities Participates in DM rounds and regularly reviews all pertinent reports as assigned by the Director of Medical affairs and Chief Medical Officer. Regularly contacts the DM patients to ensure compliance with and comprehension of treatment plans, completion of necessary referrals and testing, and scheduling of follow up PCP visits. Assists with member prioritization based on familiarity with the DM patients. Assists the DM patient’s center medical assistant with closing HEDIS gaps. Assists in monitoring and analyzing outpatient reports and identifying trends. Performs day-to-day admission review functions and general office duties including but not limited to word processing, copying, filing, faxing, answering phones and data entry. Supports the department in the development and implementation of care management initiatives. Monitors high risk/ high-cost patients in regards to care delivery, referrals, etc. Follows all regulatory policies and procedures, privacy and security standards in accordance with government agencies including HIPAA requirements. Schedule DM patients for APRN telehealth appointments. Chart prep 24 hours in advance of patient appointment. Performs other duties as assigned. Qualifications Education/Experience: High School diploma required. Minimum 3 years of related experience in a clinic or hospital setting. Knowledge of health care and managed care administration policies. Must hold a medical assistant certification from an accredited organization. Must be proficient with MS Office programs. Skills: Use relevant information and individual judgment to determine whether events or processes comply with laws, regulations, or standards. Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines and standard accepted practices. Use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. Ability to work with individuals within and outside the organization, in professional and courteous manner. Must develop constructive and cooperative working relationships with others. Understand written sentences and paragraphs in work related documents. Strong ability to use independent judgment and initiative. Organized and able to manage competing priorities. Resourcefulness in problem solving. Actively look for ways to help people. Strong written and verbal communications skills. Knowledge of medical terminology. Must be extremely detail oriented. Strong organizational and interpersonal skills. Knowledge of EHR. Bilingual skills preferred (English/Spanish or English/Creole). Must develop constructive and cooperative working relationships with others. Foreign Doctor preferred.
