join us.Job SummaryThe Director of Cano at Home must be an RN or
above, he will oversee a team of Cano at Home clinicians in a given
region and/market to ensure provision of high-quality health care
services including; home visits, assessment, treatment plans,
hospital follow up, medication reconciliation, and clinical
intervention for medical urgencies/chronic conditions This position
will work collaboratively with other departments to meet program
goals, performance targets, and ensure the organization is well
managed in its entirety as it relates to the program
initiatives.Duties & ResponsibilitiesEssential Duties &
Responsibilities
- Essential Duties & Responsibilities
- Responsible for providing support and direct supervision to
Cano at Home clinicians.
- Responsible for providing on-site evaluations to staff on a
quarterly basis.
- Develop and maintain clinical protocols for Cano at Home
- Responsible for hiring, scheduling, evaluating, ongoing staff
development, team meetings, and individual supervisory
meetings.
- Provide on-call support as scheduled.
- Assist with periodic audits to monitor quality indicators and
ensure performance metrics are being met.
- Monitor and develop reports on Cano at Home provider
productivity.
- Provide primary care and preventive care to all members
including meeting all requirements for submission of procedure
codes related to attaining 5 stars (HEDIS measures)
- Responsible for managing supplies and resources
(transportation, medical supplies, company credit card, etc.) for
Cano at Home associates.
- Responsible for overseeing Cano at Home Wound care program
progress notes.
- Clear and concise clinical documentation to include all
diagnosis codes using ICD-10 with emphasis on active problems.
- Ensure staff provide timely visits to high-risk patients and
completing clinical documentation requirements are being met.
- Monitor clinical documentation completion and timely closure of
staff notes.
- Participate and facilitate in staff, team, and vendor
meetings.
- Assist with departmental initiatives and implementation of new
programs as they relate to Care Management and Cano at Home.
- Monitor and track effectiveness of program performance to
monitor success of key performance indicators.
- Support management to identify areas of non-compliance and
assure implementation of corrective action plans to address
deficiencies in regions.
- Collaborate with all members of our interdisciplinary care team
to deliver the highest level of care for all our patients.
- Communicate with and counsel team members on treatment options
and assist in transition of care.
- Work closely with care managers to ensure that all our members'
needs are addressed timely and cost effectively.
- Partner with the patient's primary care provider and
specialists to deliver integrated care.Supervisory Responsibilities
- Provide supervision and support to the Cano at Home, including
performance management and adherence to team policies, processes,
and workflows.Critical Results
- CR #1: --- 6 home visits/day with clinical documentation
accuracy - Cano at Home provider will complete --- 6 home
visits/day for eligible patients with --- 95 % clinical
documentation accuracy.
- CR #2: > Admits/1000: Target --- 200 - Cano at home
intervention will impact admit and er visit rate/100 in applicable
region/market.
- CR #3: Cano at Home Implementation and/or Integration -
Successful implementation and integration of Cano at Home staff and
workflows in new markets within the first 180 days.Best Practices
- Monitor team performance through on-going evaluation and
tracking of performance metrics as they relate to visits per day
and clinical documentation.
- Monitor team performance through on-going evaluation and
tracking of performance metrics as they relate to preventable ER
visits and/or inpatient admissions.
- Identify adverse trends with to proactively implementing
effective strategies that address areas for performance improvement
with clinicians not meeting performance metrics.
- Phone call audits, documentation audits
- Analyze data from electronic medical record system to ensure
outcomes are aligned with team metrics.
- Provide input into the annual review of policies and
procedures, the CM program results and outcomes, and future program
goals and interventions.
- Oversee staff productivity and ensure results are aligned with
metrics and deadlines.
- Evaluate findings prioritized for resolution and/or
improvement, develop appropriate strategies and measures for
effectiveness.
- Support training for current and/or new acquisition staff to
apply the correct workflows to effectively achieve department and
organizational metrics.Education & Experience
- Master's Degree in Nursing
- 3+ years' direct patient care experience
- Foreign Medical Graduate (FMG) preferred.
- Must have a valid Nursing License.
- Nurse Practitioner License Preferred
- Must be familiar with the EHR medical charting system.
- Strong written and interpersonal communication skills
- Experience with eCW highly preferred
- Must have experience with managed-care
- Bilingual: English and Spanish requiredKnowledge, Skills &
Proficiencies
- Advanced skills with Microsoft Office required.
- Excellent organizational, interpersonal, and verbal and written
communication skills required; ability to attend to detail,
organize tasks/priorities and complete with accuracy.
- Ability to pay close attention to detail and to ensure accuracy
of reports and data.
- Critical thinking, creativity, and flexibility with an ability
to adapt to various and fast paced environments.
- Exceptional interpersonal skills with ability to interface
effectively both internally and externally with a wide range of
people including physicians, office staff and other health plan
staff.
- Ability to act quickly, assess and act accordingly in crisis
situations.
- Skill in operating phones, personal computers, software, and
other IT systems.
- Must have a valid driver's license and be able to drive to
conduct field visits.
- Must have ability to analyze and solve problems.

RequirementsPhysical RequirementsThis position works from corporate
location along with hybrid when conducting audits during field
visits with the team. The employee is required to work at a
personal computer as well as be on the phone for extended periods
of time. Must be able to stand, sit, walk, and occasionally climb.
The incumbent must be able to work extended and flexible hours and
weekends as needed. Physical demands include the ability to lift to
