Position Purpose:Employment Type: Full time Shift:Description: The goal of the Acute Care Manager is to coordinate and manage care to assist patients in meeting their multiple needs, ensure handover and continuity of services, and to decrease fragmentation of care and services. Works collaboratively with members of the Interprofessional acute care, ambulatory and post-acute care management teams to improve communication across the continuum of care.Quality* Ensures appropriate clinical documentation to support level of care. Analyzes the patient's current clinical status, treatment plan and past medical history to identify potential gaps on physician documentation. Identifies the principal diagnosis and all secondary conditions, co-morbidities, complications and procedures that have been documented in the medical record.* Serves as a consultant and educator with medical staff regarding status, placement, payer requirements, and utilization of resources.* Assures appropriate sequencing of care/interventions for optimal results and smooth transition along hospital care and continuum.* Proactively identifies and removes barriers that impede the progress of care. Coordinates level of care appropriateness with admission status determination.* Assures clear admission order. Meets and acts on cases not meeting admission criteria. Anticipates principle diagnosis and procedures and appropriately assigns length of stay.* Maintains current knowledge of basic health care economics, trends and reimbursements methodologies and applies knowledge to daily practice. Recognizes situations that require immediately intervention/ Continues to develop professionally and apply knowledge and skills necessary to provide care appropriate to each patient.* Accountable to organizational goals and metrics such as but not limited to O/E Ratio, LOS, Patient Satisfaction, Joint Commission readiness and Quality metrics.Daily Operations* Actively triages patients currently present on the unit for appropriateness of care.* Reviews LOS values and working DRG daily; Updates in CareLogistics/EPIC software as needed.* Coordinates Physician Advisor referrals when there are concerns regarding meeting level of care criteria. Reviews PA referrals with department manager.* Assesses the health care, educational, and psychosocial needs of the patient/family. Uses standardized assessment tools such as AMPAC functionality, Palliative Screening and other screening to assist with this process. Facilitates Advanced Care Planning Discussions.* Links patient to community and social support resources (CHWB referral)* Monitors anticipated and pending discharges. Collaborates with nurses, ancillary services, family physicians and all others needed to ensure timely discharge of patients.* Identifies Advanced Patient Management/Alternate Payment Model (APM) patients, readmitted and high-risk for readmission patients. Collaborates with Population Health Nurse, COPD Navigators, CHF Nurse and Coordinator, and other healthcare providers for a safe transition to the Least Restrictive Next Site of Care (NSOC).* Ensures the elements of the Readmission Playbook are met for all readmitted or patients at high risk for readmission patients including notifying the pharmacist to review medications once discharge is pending/written and before the patient is transitioned.* Coordinates simple discharge planning, i.e., DME, transportation. Coordinates discharge plan related to patient needs and payer requests. Delegates appropriately to the social worker for complicated discharge planning.* Identifies potential cases for initiation of notices of non-coverage and advance beneficiary notice. Initiates condition code 44 and hospital requested reviews when appropriate.* Accountable to lead SNAP huddles and that all components are met.* Works with unit staff to ensure EPIC/Care Logistics software accurately reflects unit and patient status (anticipated transfers, discharges).* Queries the physician, residents / interns, mid-level clinicians and other clinicians for plan of care and anticipated time of discharge, based on current clinical findings, prescribed treatment, medical interventions and / or procedures.* Participates in hospital and/or department committees or teams as requested or appointed by the department manager.* Performs all other duties as assigned.Trinity Health's Commitment to Diversity and InclusionTrinity Health employs about 133,000 colleagues at dozens of hospitals and hundreds of health centers in 22 states.
